O panis suavissime
Fidelium refectio,
Merum o nectarium

Munda cordis oculum
Salva cunctum populum
Per salutis poculum.

 

English translation: 

O sweetest Jesus, o most delightful bread, refreshment of the faithful, o unmixed wine among the wines of the gods.

Purge the eye of the heart, save all the people through the chalice of salvation.
